What is the Cardiac Rehabilitation Program?

Cardiac rehabilitation is an education and exercise program that helps people gain information about how to manage their disease through changing their daily health habits.

Horizon's health care professionals such as nurses, physiotherapists, dietitians and pharmacists are available to provide the information you need to recover from a heart attack, other forms of heart disease or surgery to treat heart disease.

How is this service provided?

This free program runs over a period of generally 12 weeks and involves supervised exercise at a facility or a supervised home exercise program.

Classes are held to provide information about heart disease and how to improve risk factors associated with heart disease.

How can I access this program?

The program is open to people who have heart disease and need help in adapting heart healthy behaviours.

Your family physician or cardiologist can refer you to the program. Once the referral is received, a health care professional will contact you to set up an initial visit.

Your physician or cardiologist can refer you to the program either before you leave the hospital or after you have been discharged.

The referral is sent to a health care provider who will review the information. The health care provider will contact you to set up an appointment. At this first appointment you will meet with a nurse or physiotherapist who will check your heart rate, blood pressure, listen to your heart and lungs, and assess your current level of fitness.

Depending upon how fit you are, they will recommend different exercise options for you. The program requires a 12-week commitment from participants.

You should wear comfortable, non-restrictive clothes.
