In Your Community highlights Horizon’s partners at work
(HORIZON) Thursday, Sept. 26, 2019 - Two of Horizon Health Network's (Horizon) rural communities are helping people be healthy by educating community members on health care services available to them and empowering clients.
The family support coordinator in the Sussex and Surrounding Area connects individuals to services and programs, promotes wellness, provides healthy growth strategies and helps clients and their families navigate health and social systems.
In the Tobique and Perth-Andover Area, Horizon staff are delivering health-related presentations to ensure patients and clients know what programs and services are available to them right in their community and how to access them.
These are just two of the stories featured in the seventh, and final edition ofIn Your Community: Community Health Needs Assessments in Action. This publication highlights the achievements stemming from Community Health Needs Assessment (CHNA) recommendations across Horizon.
Karen McGrath, President and CEO of Horizon, said she is always impressed by the resiliency of communities and the ability to come together to identify solutions that overcome challenges.
"In each edition ofIn Your Community, it is very clear to me that community strength is at the heart of each success story," said McGrath. "It is this strength will continue to serve as motivation for future health care improvements."
Jean Daigle, VP Community for Horizon, said CHNAs provide Horizon and communities with information on how to improve population health.
"With the second round of CHNAs underway, we are already identifying and evaluating changes in the priorities for our communities since the last assessments were completed," said Daigle. "These important stories highlight a communities' strengths and innovation when it comes to helping people be healthy."
Daigle said while this will be the final publication, Horizon is embarking on a new and innovative way to share its success stories.
"Soon, you'll be reading these stories on Horizon's Community News Channel and on all Horizon's social media platforms," Daigle said.
