Annual ‘REACH’ Conference Promotes Patients as Partners in Health Care

(Moncton) May 25, 2015 - Health-care professionals from across the Maritimes will come together for the annual 'REACH' conference on May 26 and 27 at the Delta Beauséjour in Moncton. This year's conference will emphasize "Patients as Partners - Supporting Patients with Chronic Conditions through Effective Collaboration."

"Chronic health conditions deeply affect day-to-day life for patients and their families. Many people have more than one condition and have multiple health care providers. Our goal this year is to demonstrate how inter-professional teams and community partners can work together and support patients and their families through effective teamwork and communication," said Denise Connors, Manager, Chronic Disease Prevention and Management with Horizon Health Network and conference co-chair. "We want to ensure that patients are informed and included in decisions about their health care."

The conference will welcome award winning health journalist, author and patient safety advocate, Suzanne C. Gordon as the keynote speaker. Gordon will also present "Bedside Manners", an interactive play that focuses on communication, patient safety and teamwork featuring local health-care workers as the actors.

"Bedside Manners" has been presented at top health care institutions across North America and will be performed on Tuesday, May 26 at 6:45 p.m. The cost for non-conference participants is $15/person and includes a reception featuring food and beverages from local producers. The public is welcome to attend the performance and to register for the conference.

In an effort to promote a healthy and active lifestyle the REACH conference proudly offers opportunities for physical activity and healthy meals and snacks. Thanks to the unique partnership between Horizon and the Delta, the menu will focus on supporting local food producers.
